(1)   Is licensed to practice dental hygiene in any
other state or any province of Canada;

(2)  Meets any other qualifications that the Board
establishes;

(3)  Submits to the Board an application on the form
that the Board requires; and

(4)  Pays to the Board an application fee [of $10] SET
BY THE BOARD.

4.5-206.

(a) [Except for the fees specifically set under this title,
the] THE Board may set reasonable fees for the issuance and
renewal of licenses.

4.5-308.

(c) Before the license expires, the licensee periodically
may renew it for an additional 2-year term, if the licensee:

(1)  Otherwise is entitled to be licensed;

(2)  Pays to the Board [the] A renewal fee [of $25 per
year] SET BY THE BOARD; and

(3)  Submits to the Board:

(i) A renewal application on the form that the
Board requires; and

(ii) Satisfactory evidence of compliance with
the continuing education requirements for license renewal set by
the Board under this section.

(a) (1) [Except for the fees specifically set by this
title, the] THE Board may set reasonable fees for the issuance
and renewal of licenses and its other services.

5-303.

To apply for a license, an applicant shall:

(1)  Submit to the Board an application on the form
that the Board requires;

(2)  Submit to the Board evidence of compliance with
the requirements of § 5-302 of this subtitle; and
