Ch. 3

(C) CONDITIONS.

THE STATE BOARD SHALL GRANT A WAIVER UNDER THIS SECTION FOR
AN APPLICANT WHO HOLDS A LICENSE FROM A LOCAL JURISDICTION ONLY
IF THE APPLICANT:

(1)  (I) 1. QUALIFIED FOR THE LICENSE BY PASSING AN
EXAMINATION GIVEN IN THE LOCAL JURISDICTION; AND

2. HAS HELD THE LICENSE FOR AT LEAST 1
YEAR IMMEDIATELY BEFORE THE DATE OF APPLICATION; OR

(II) 1. QUALIFIED FOR THE LICENSE OTHER THAN BY
PASSING AN EXAMINATION IN THE LOCAL JURISDICTION; AND

2. HAS HELD THE LICENSE FOR AT LEAST 2
YEARS IMMEDIATELY BEFORE THE DATE OF APPLICATION;

(2)  SUBMITS A WRITTEN STATEMENT FROM THE LOCAL
JURISDICTION CERTIFYING:

(I)   IF THE LICENSE IS ISSUED ON THE BASIS OF AN
EXAMINATION, THE DATE OF ISSUANCE OF THE LICENSE; AND

(II)  THE LICENSEE:

1.   IS IN GOOD STANDING WITH THE LOCAL
JURISDICTION; AND

2.  HAS NO RECORD OF OUTSTANDING
VIOLATIONS OF REGULATIONS OF THE LOCAL JURISDICTION;

(3)  PAYS ANY RECIPROCAL LICENSE FEE REQUIRED UNDER
THIS TITLE;

(4)  IF REQUIRED, SUBMITS PROOF OF GENERAL LIABILITY
INSURANCE TO THE STATE BOARD; AND

(5)  COMPLIES WITH ANY OTHER LICENSING REQUIREMENTS OF
THE STATE BOARD.
