(A) TERM OF STATE LICENSE. UNLESS A STATE LICENSE IS RENEWED FOR A 2-YEAR TERM AS (1) AFTER THE EFFECTIVE DATE OF THE STATE LICENSE; (2) IN AN ODD-NUMBERED YEAR. (B) RENEWAL NOTICE. AT LEAST 1 MONTH BEFORE A STATE LICENSE EXPIRES, THE STATE (1) A RENEWAL APPLICATION FORM; AND (2) A NOTICE THAT STATES: (I) THE DATE ON WHICH THE CURRENT STATE LICENSE (II) THE DATE BY WHICH THE STATE BOARD MUST (III) THE AMOUNT OF THE RENEWAL FEE.
