(3) Anyone who lives in the same house as the
individual; or

(4) Any physician.

(b)  A petition under this section may be filed with the
district court in or circuit court for the county where the
[alleged drug addict] INDIVIDUAL ALLEGED TO HAVE A DRUG DEPENDENCE
resides or otherwise may be found.

(c)  A petition under this section shall:

(1)  Be signed and verified by the petitioner;

(2)  State the petitioner's:

(i) Name;

(ii) Title;

(iii) Address; and

(iv) Relationship to the [alleged drug addict]
INDIVIDUAL ALLEGED TO HAVE A DRUG DEPENDENCE;

(3)  State the [alleged drug addict's] INDIVIDUAL'S
ALLEGED TO HAVE A DRUG DEPENDENCE:

(i) Name;

(ii) Address;

(iii) Telephone number, if any;

(iv) Birth date;

(v) Birth place;

(vi) Age;

(vii) Sex;

(viii) Marital status;

(ix) Occupation; and

(x) Physical description;

(4)  Contain statements that support the belief that
the [alleged drug addict is a drug addict] INDIVIDUAL ALLEGED TO
HAVE A DRUG DEPENDENCE HAS A DRUG DEPENDENCE; and
