provided;

4.  How services will be

coordinated with those

provided by the Health

Education and Advocacy Unit in
the Attorney General's Office;

5.  How consumers will gain access
to the program;

6.  The budget for the program,
including any local match and
private contributions;

7.  A timetable for statewide
implementation;

8.  The anticipated quantity of
various types of services to
be provided (e.g., number of
people counseled, number of

educational sessions

provided).

The plan should be submitted to the
Department of Fiscal Services by
September 1, 1987................
