Ch. 670 LAWS OF MARYLAND (b) An applicant who is an individual, and any individual (1) At least 18 years old; and (2) Of reputable and responsible character. (C) (1) [The] EXCEPT FOR A LIMITED LICENSEE, THE applicant (2) The hospice CARE PROGRAM to be operated and its 19-907. (a) An applicant for a license shall: (1) Submit an application to the Secretary; and (2) Pay to the Secretary an application fee [of $10] (b) The application: (1) Shall be on the form that the Secretary requires; (2) Shall be signed and verified by the individual; (3) Shall include: (i) The name of the applicant; (ii) A statement that the applicant meets the (iii) The location of the proposed hospice; .
