(c)  Before the [certification of a registered environmental
sanitarian] LICENSE expires, the [registered environmental
sanitarian certified] LICENSEE may renew it for an additional
2-year term, if the [registered environmental sanitarian]
LICENSEE:

(1)  Otherwise is entitled to be [certified] LICENSED;

(2)  Pays to the Board [a] THE renewal fee SET BY THE
BOARD;

(3)  Submits to the Board a renewal application on the
form that the Board requires; and

(4)  Submits to the Board proof that during the
previous 2-year period, [he or she has informed themselves of
developments in the field of environmental health by pursuing]
THE LICENSEE HAS ACQUIRED 20 hours of approved training IN
ENVIRONMENTAL HEALTH or other [means determined] equivalent
EDUCATION AS APPROVED by the Board.

(d)  The renewal [certificate] LICENSE shall bear the same
serial number assigned to the [registered environmental
sanitarian] LICENSEE at the time of the original [certification]
REGISTRATION OR LICENSURE.

5.5-310.

The Board shall reinstate the [certification] LICENSE of a
registered environmental sanitarian who has failed to renew the
[certification] LICENSE for any reason if the registered
environmental sanitarian:

(1)  Pays the Board all lapsed renewal fees AND
DEMONSTRATES THAT TRAINING AS REQUIRED BY THE BOARD HAS BEEN
COMPLETED;

(2)  Reapplies and meets the qualifications and
requirements for [certification] LICENSURE; or AND

(3)  Pays to the Board a reinstatement fee set by the
Board.

5.5-311.

(a)  The Board shall keep a current record of each
application for [certification] LICENSURE.

(b)  The record shall include:

(1)  The name, residence address, and age of each
applicant;

(2)  The name and address of the applicant's employer;
