(a)  Certification as a sanitarian in training expires on
the fourth anniversary of its effective date, unless it is
renewed for a 4-year term as provided in this section.

(b)  At least 1 month before the certification as a
sanitarian in training expires, the Board may send to the
sanitarian in training certified, by first class mail to the last
known address of the sanitarian in training, a renewal notice
that states:

(1)  The date on which the current certification
expires;

(2)  The date by which the renewal application must be
received by the Board for the renewal to be issued and mailed
before the certification expires; and

(3)  A renewal fee of $10.

(c)  The Board may renew the certification of a sanitarian
in training.]

The Board shall reinstate the certification of a registered
[sanitarian] ENVIRONMENTAL HEALTH PRACTITIONER SANITARIAN who has
failed to renew the certification for any reason if the

registered [sanitarian] ENVIRONMENTAL HEALTH PRACTITIONER

SANITARIAN:

(1)  Pays the Board all lapsed renewal fees; or

(2)  Reapplies and meets the qualifications and
requirements for certification.

(a)  The Board shall keep a current record of each
application for certification.

(b)  The record shall include:

(1)  The name, residence address, and age of each
applicant;

(2)  The name and address of the applicant's employer;

(3)  The date of the application;

(4)  Complete information on the education and
experience qualifications of each applicant;

(5)  The date the Board reviewed and acted on the
