HEALTH;
4. THE COUNTY ATTORNEY FOR MONTGOMERY
COUNTY;
5. THE MARYLAND STATE POLICE; OR
6. ANY DESIGNATED OFFICER OR AGENT OF THE
STATE AND LOCAL GOVERNMENT OFFICES AND AGENCIES ENUMERATED IN
THIS SUBPARAGRAPH.
(B) (1) AN INDIVIDUAL OPERATING THE GAME OF BINGO SHALL BE
A RESIDENT OF MONTGOMERY COUNTY AND A MEMBER OF THE NONPROFIT
ORGANIZATION.
[(b) (1) An] (2) THE NONPROFIT organization desiring to
conduct a game of bingo in Montgomery County shall first secure a
bingo license from [the county department of health] MONTGOMERY
COUNTY. LICENSE [Application] APPLICATION shall be made on
forms provided by [the department] MONTGOMERY COUNTY and shall
disclose the following information with respect to the NONPROFIT
organization:
[(1)] (I) The name of the NONPROFIT organization and
a list of the names and addresses of its officers and directors;
[(2)] (II) A full and complete statement of the
purposes and objectives of the NONPROFIT organization and the
purposes for which the proceeds from [a] THE bingo operation will
be used;
[(3)] (III) A statement under affidavit by the
president and treasurer of the NONPROFIT organization, or by its
chief executive and fiscal officer that 1. no agreement exists
for the diversion of any portion of the proceeds of [a] THE bingo
operation to any other person, [partnership, or corporation,] OR
LEGAL OR BUSINESS ENTITY and 2. no person, [partnership,
organization, or corporation] OR LEGAL OR BUSINESS ENTITY shall
receive any portion of the proceeds of the bingo operation except
in furtherance of the purpose of the NONPROFIT organization; AND
[(4)] (IV) Any additional information [the department
of health] MONTGOMERY COUNTY may require.
[(c)] (3) An annual bingo license for the operation of the
game of bingo known as an "annual bingo license" shall authorize
the holder to operate a game of bingo at a specified fixed
location at any time during the year for which the license is
