(1)  That the' violation of [subsections] SUBSECTION
(a) or (b) of this section was knowing and willfull [and,]; AND (2)  That the violation of subsection (a) or (b) OF
THIS SECTION was the basis for the surety's action. 414. (k-1) (6) The recalculated nonforfeiture net level premium
shall be equal to the result obtained by dividing THE SUM IN
SUBPARAGRAPH (i) IN THIS PARAGRAPH by THE VALUE IN SUBPARAGRAPH
(ii) [where:] IN THIS PARAGRAPH. (i) Equals the sum of 1.   The nonforfeiture net level premium
applicable prior to the change times the present value of an
annuity of 1 per annum payable on each anniversary of the policy
on or subsequent to the date of the change on which a premium
would have fallen due had the change not occurred; and 2.   The present value of the increase in
future guaranteed benefits provided for by the policy, and (ii) Equals the present value of an annuity of
1 per annum payable on each anniversary of the policy on or
subsequent to the date of change on which a premium falls due. 424. The lives of a group of individuals may be insured under a
policy issued to a credit union organized pursuant to the laws of
Maryland or the Federal Credit Union Act, which shall be deemed
the policyholder, to insure the members of the credit union, for
the benefit of persons other than the credit union or its
officials, subject to the following requirements: (3)  The amount of insurance on the life of any member
may not [time] exceed the total amount of his shares and deposits
in the credit union, or $5,000 whichever is less. Article 56 - Licenses 191. (m) In Anne Arundel County, the fees for dog and kennel
licenses shall be determined by the County Council of Anne
Arundel County. A dog license shall expire when a rabies
vaccination certificate issued under § 18-319(a)(3) of the
[Health] HEALTH - General Article expires. 336. (c) Certificates of registration shall expire on the last
