Annotated Code of Maryland
(1974 Volume and 1982 Supplement) SECTION 1. BE IT ENACTED BY THE GENERAL ASSEMBLY OF
MARYLAND, That Section(s) 4-401(k) through (n) and 7-201(h)
through (g), respectively, of the Health - Environmental Article,
of the Annotated Code of Maryland be renumbered to be Section(s)
4-401(1) through (o) and 7-201(i) through (r), respectively. SECTION 2. AND BE IT FURTHER ENACTED, That the Laws of
Maryland read as follows:
Article - Health - General
4-214.
(c) (1) On receipt of a court order that changes the name
of an individual who was born in this State and on request of the
individual or a parent, guardian, or legal representative of the
individual, the Secretary shall amend the certificate of birth to
reflect the new name[, however, the]. (2) THE Department [of Health and Mental Hygiene] may
change the name on a birth certificate once without a court order
if, within 6 months [of] AFTER the birth, the Department receives
FROM BOTH PARENTS OF A CHILD: [(l)] (I) A written request [from both parents
requesting] FOR the change of name; and [(2)] (II) An affidavit [from both parents]
THAT HAS BEEN sworn before a notary public of [the] THIS State
[of Maryland stating] AND STATES that they are the [true] parents
of the child and are making this request of their own free will.
