(2)  IDENTIFY THE INFECTIOUS OR CONTAGIOUS
DISEASE;

(3)  STATE THE NAME OF THE ADMINISTRATIVE HEAD OF
THE INSTITUTION; AND

(4)  STATE THE ADDRESS OF THE INSTITUTION.

REVISOR'S NOTE: Subsection (a) of this section is new
language added to avoid the long phrase
"boardinghouse, lodging house, ... or ...
institution of any kind".

Subsections (b) and (c) of this section are new
language derived without substantive change from
the first and second clauses of former Article
43, § 100.

Throughout this section, the word "report" is
substituted for the word "notice", to conform to
Department practice.

In subsections (b) and (c)(3) of this section,
the references to an "administrative head" of an
institution are substituted for the enumerations
of types of persons in charge of institutions, to
conform to similar references elsewhere in this
article.

Also in subsection (b) of this section, the
former references to a "guest" or "inmate" are
deleted as unnecessary in light of the broad
reference to individuals "on the premises".

Also in subsection (b) of this section, the
former enumeration of types of infectious and
contagious diseases is deleted as unnecessary.
See revisor's note to § 18-201 of this subtitle.

In subsection (c)(1) and (4) of this section, the
terms "residence address" and "address" are
substituted for the terms "place of residence"
and "locality", for clarity and conformity to
Department practice.

The third clause of former Article 43, § 100,
which required a health officer to "render
effective the provisions of §§ 55 to 64,
inclusive", is deleted as unnecessary as to
former Article 43, §§ 55, 57, 58, 59, and 62, in
light of the revision of those sections in §§
18-210, 18-211, 18-501, 18-502, and 5-506 of this
article, respectively. That former clause is
unnecessary as to former Article 43, §§ 56, 60,
