(1)  BE SIGNED AND VERIFIED BY THE PETITIONER;

(2)  STATE THE PETITIONER'S:

(I)  NAME;

(II)  ADDRESS;

(III)  TELEPHONE NUMBER, IF ANY;

(IV)  BIRTH DATE;

(V)  BIRTH PLACE;

(VI)  AGE;

(VII)  SEX;

(VIII)  MARITAL STATUS;

(IX)  OCCUPATION; AND

(X)  PHYSICAL DESCRIPTION;

(3)  CONTAIN STATEMENTS THAT SUPPORT THE BELIEF
THAT THE PETITIONER IS A DRUG ADDICT; AND

(4)  CONTAIN A STATEMENT THAT THE PETITIONER
NEEDS CARE, SUPERVISION, AND TREATMENT.

REVISOR'S NOTE: Subsections (a) and (b) of this section
are new language derived without substantive
change from the first sentence of former Article
43B, § 13.

Subsection (c) of this section is new language
added to set forth the substance of former
Article 43B, § 9(c)(2), (3), and (4), to reflect
that those provisions apply to a proceeding under
Part V of this subtitle. See revisor's note to §
9-601 of this subtitle.

Throughout this section and Part V of this
subtitle, the former references to a "penal ...
institution" are deleted as unnecessary in light
of the broad reference to a "correctional
institution".

In subsection (a) of this section, the reference
to the "Maryland Parole Commission" is
substituted for the obsolete reference to the
former State Parole Board.
