individual who is sentenced but not yet serving
it, since Part IV only deals with an individual
convicted but not yet sentenced and Part V deals
with an individual who is actually serving a
sentence.

9-631. PETITION.

A PETITION UNDER THIS SECTION SHALL:

(1)  BE SIGNED AND VERIFIED BY THE PETITIONER;

(2)  UNLESS THE PETITIONER IS THE ALLEGED DRUG
ADDICT, STATE THE PETITIONER'S:

(I)  NAME;

(II)  TITLE;

(III)  ADDRESS; AND

(IV)  RELATIONSHIP TO THE ALLEGED DRUG
ADDICT;

(3)  STATE THE ALLEGED DRUG ADDICT'S:

(I)  NAME;

(II)  ADDRESS;

(III)  TELEPHONE NUMBER, IF ANY;

(IV)  BIRTH DATE;

(V)  BIRTH PLACE;

(VI)  AGE;

(VII)  SEX;

(VIII)  MARITAL STATUS;

(IX)  OCCUPATION; AND

(X)  PHYSICAL DESCRIPTION;

(4)  CONTAIN STATEMENTS THAT SUPPORT THE BELIEF
THAT THE ALLEGED DRUG ADDICT IS A DRUG ADDICT; AND

(5)  CONTAIN A STATEMENT THAT THE ALLEGED DRUG
