(4)  THE NAME AND BUSINESS ADDRESS OF THE SELLER.

REVISOR'S NOTE: This section is new language derived
without substantive change from former Article
43, § 275, as that section related to the
contents of a label for controlled poison.

"In addition to any other requirement of law" is
new language added for clarity. See, e.g., §§
5-201 and 5-202 of this title.

In item (2) of this section, the word "substance"
is substituted for "article" for clarity.

5-115. POISON REGISTER.

(A)  SCOPE OF SECTION.

THIS SECTION DOES NOT APPLY TO THE DISPENSING OF A
CONTROLLED POISON ON THE PRESCRIPTION OF AN AUTHORIZED
PRESCRIBER IF THE QUANTITY OR DOSE IS NOT UNUSUAL.

(B)  REQUIRED RECORDS.

A PERSON WHO SELLS, DELIVERS, OR PROVIDES ANY
CONTROLLED POISON SHALL RECORD IN A POISON REGISTER:

(1)  THE DATE OF SALE OR DELIVERY;

(2)  THE NAME AND ADDRESS OF THE PERSON FOR WHOM
THE CONTROLLED POISON IS PROVIDED;

(3)  THE NAME AND QUANTITY OF THE CONTROLLED
POISON;

(4)  THE PURPOSE FOR WHICH THE CONTROLLED POISON
IS TO BE USED;

(5)  THE NAME OF THE DISPENSER; AND

(6)  THE SIGNATURE OF THE INDIVIDUAL WHO RECEIVES
THE CONTROLLED POISON.

(C)  SUBSTITUTE RECORD.

IF A CONTROLLED POISON IS SOLD, DELIVERED, OR PROVIDED
TO AN INDIVIDUAL UNDER THE AGE OF 15 YEARS, THE WRITTEN
ORDER REQUIRED BY § 5-113(C) OF THIS SUBTITLE SHALL BE
