(3) AT A MINIMUM:

(I)  IDENTIFIES CLEARLY EACH PATIENT BY:

1.  NAME;

2.  NUMBER;

3.  AGE; AND

4.  SEX; AND

(II)  SHOWS CLEARLY:

1.  THE SERVICES PROVIDED;

2.  WHEN THE SERVICES ARE PROVIDED;

3.  WHERE THE SERVICES ARE PROVIDED;

4.  BY WHOM THE SERVICES ARE

PROVIDED;

5.  THE DIAGNOSIS AND PROGNOSIS, IF

APPROPRIATE;

6.  THE TREATMENT;

7.  ANY DRUG THERAPY; AND

8.  THE HEALTH STATUS OF THE PATIENT,
IF APPROPRIATE.

REVISOR'S NOTE: This section formerly appeared as
Article 43, § 844(a)(1) through (11).

In subsection (i)(3) of this section, the
reference to the "Department of Health and Human
Services" is substituted for the reference to the
"Department of Health, Education and Welfare", to
conform to current nomenclature.

In subsection (k) of this section, the reference
to "all other matters" is substituted for
"financial matters", to conform to the broad
scope of the Commissioner's authority.

The only other changes are in style.

As to subsections (e) and (i) of this section,
the references to "Title XVIII or Title XIX of
the Social Security Act" refer to Subchapter
XVIII - Health Insurance for the Aged and
Disabled (Medicare) in 42 U.S.C. § 1395 et seq.
and Subchapter XIX - Grants to States for Medical
