requiring registration under this subtitle, [whichever is
later,] every person required to register with the
Commission pursuant to subsection (a) shall provide the
following information, where appropriate, on a form provided
by the Commission:

(1)  The registrant's name and permanent address;

(2)  The name and permanent address of each
person who is required to register under this section and
who will lobby on the registrant's behalf;

(3)  The name, address, and nature of business of
any person who compensates the registrant accompanied by the
written authorization required by § 5-102 and a statement
whether that person will be exempt from the provisions of
this title, pursuant to § 5-101(b), because of the filing
and reporting of the registrant; and

(4)  The identification, by formal designation,
if known, of matters on which the registrant expects to act
or employ someone to act in a manner which requires
registration under this title.

(c)  A registrant shall file a separate registration
for each employer.

(d)  Each registrant may file a notice of termination
within 30 days after ceasing any activity that requires
registration and filing the reports required hereunder.

5-104. Compensation not to be contingent.

A person may not be employed as a registrant for
compensation dependent in any manner upon the passage or
defeat of any proposed legislation, or upon any other
contingency connected with any action of the General
Assembly.

5-105. Reports by registrants to the Commission.

(a) Each registrant shall file with the Commission,
two reports under oath concerning his lobbying activities,
one report covering the period beginning on November 1
through April 30, filed by May 31, and one report covering
the period beginning on May 1 through October 31 filed by
November 30. If the registrant is not an individual, an
authorized officer or agent of the registrant shall sign the
form. Each registrant shall file a separate activity report
for each person from whom he receives compensation. The
report shall include:

(1)  A complete and current statement of the
information required to be supplied pursuant to § 5-103(b)
of this title;

(2)  Total expenditures on acts requiring
registration in each of the following categories:
