Ch. 688

(3) (4) A FACILITY HOME ESTABLISHED AFTER THE
EFFECTIVE DATE OF THIS SUBTITLE MUST COMPLY WITH ALL GENERAL
ZONING ORDINANCES IN THE FOLLOWING RESPECTS:

(I) THE HEIGHT AND SIZE OF THE BUILDINGS

AND STRUCTURES INVOLVED,

OCCUPIED,

SPACE,

STRUCTURES; AND

(II)   LAND WHICH MAY BE COVERED OR

(III)  AREA OR SIZE OF YARDS REQUIRING OPEN

(IV)   DENSITY OF POPULATION, AND

(V)    USAGE OF LAND, BUILDINGS, AND

(4) (5) THE FACILITY HOME HAS AGREED TO A
STANDARDIZED PROCEDURE ACCEPTABLE TO THE SECRETARY FOR THE
RECEIPT, INVESTIGATION, DOCUMENTATION, AND DISPOSITION OF
FORMAL WRITTEN COMPLAINTS FROM NEIGHBORHOOD RESIDENTS.

(A)    A   PRIVATE GROUP HOME OR LARGE PRIVATE GROUP HOME
MAY NOT BE   ESTABLISHED UNTIL AN INITIAL PERMIT AND LICENSE

APPLICATION      FOR A LICENSE HAS BEEN OBTAINED FROM THE

SECRETARY.

(B)           APPLICATION FOR AN INITIAL PERMIT AND A LICENSE
SHALL BE ON A FORM PRESCRIBED BY THE SECRETARY, SWORN TO BY
THE APPLICANT AND SHALL CONTAIN AT LEAST THE FOLLOWING
INFORMATION:

(1)          THE NAME AND ADDRESS OF THE APPLICANT;

(2)          THE LOCATION INCLUDING THE STREET NUMBER OF
THE PROPOSED FACILITY;

(3) THE NAME AND ADDRESS OF THE OWNER OF THE

PROPERTY; AND

(4) A DESCRIPTION OF THE PROGRAM PROPOSED THE

PARTICULAR___LOCATION FOR WHICH THE LICENSE IS REQUIRED,

DESIGNATING THE STREET NUMBER IF PRACTICABLE,___AND IF NOT

PRACTICABLE THEN ANY DESCRIPTION WHICH DEFINITELY LOCATES

THE PREMISES;

(3) THE NAME OF THE OWNER OF THE PROPERTY UPON

WHICH THE HOME IS PROPOSED, IF OTHER THAN THE APPLICANT;

(4) A STATEMENT THAT THE APPLICANT WILL CONFORM

TO ALL LAWS AND REGULATIONS RELATING TO__THE__ ESTABLISHMENT,

MAINTENANCE, AND OPERATION OF THE HOME;
