such certificate is not revoked or suspended as provided

[[296.

(A) AH APPLICANT FOR A LICEHSE TO PRACTICE
[PRACTICAL HORSING] AS A LICENSED PRACTICAL NURSE shall
submit to the Board evidence, verified by oath or
affirmation that the applicant[:

(1)   Is of good moral character.

(2)    Has completed at least two years of high
school.

(3)   Has] HAS completed HIGH SCHOOL AND a required
State—approved LICENSED practical nursing education
program, as defined by the Board, and for the
satisfactory completion of which holds a diploma.

(b)    The applicant [shall be] IS required to pass a
written examination in [sock] subjects [as] DETERMINED BY
the Board [may determine]. Upon successful completion of
such examination the Board shall issue to the applicant a
license to practice as a licensed practical nurse.

[c)    The Board say issue a license to practice
[practical nursing} AS A LICENSED PRACTICAL NURSE without
examination by endorsement to any applicant who has been
duly licensed or registered as a licensed practical nurse
or a person entitled to perform similar services under a
different title, under lacs of another state, territory
or foreign country, if, in the opinion of the Board, the
applicants educational qualifications at the time of his
or her graduation fulfilled the requirements of this
State [at such time].

[(d) Application for license under this subsection
(d) must be made before June 1, 1969. The Board may
issue a license to practice as a licensed practical nurse
to any person who shall submit to the Board written
evidence, verified by oath or affirmation, that said
applicant:

(1)   Is of good moral character.

(2)   Has practiced practical nursing in this State
for at least two years within the five—year period
immediately preceding June 1, 1967; provided such person
passes a written examination administered by the Board.}

[(e)] (D) The applicant applying for a license to
practice as a licensed practical nurse shall pay the fee
