(A)   COMPLETION OF JUROR QUALIFICATION FORM.

THE CLERK OR JURY COMMISSIONER SHALL MAIL TO EVERY
PERSON WHOSE NAME IS DRAWN FROM THE MASTER WHEEL UNDER
§8-205 A JUROR QUALIFICATION FORM, WITH INSTRUCTIONS
TO FILL OUT, SIGN, CERTIFY, AND RETURN IT BY MAIL TO
THE CLERK OR JURY COMMISSIONER WITHIN TEN DAYS.

(1)   IF THE PERSON IS UNABLE TO FILL OUT THE
FORM, HE SHALL HAVE ANOTHER PERSON DO IT FOR HIM, AND
THAT PERSON SHALL INDICATE THAT HE HAS DONE SO, AND
THE REASON.

(2)   IF IT APPEARS THAT THERE IS AN OMISSION,
AMBIGUITY, OR ERROR IN A FORM, THE CLERK OR JURY
COMMISSIONER SHALL RETURN THE FORM WITH INSTRUCTIONS
TO THE PERSON TO MAKE THE ADDITIONS OR CORRECTIONS
NECESSARY AND TO RETURN THE FORM TO THE CLERK OR JURY
COMMISSIONER WITHIN TEN DAYS OF RECEIPT.

(B)   FAILURE TO RETURN FORM.

ANY PERSON WHO FAILS TO RETURN A COMPLETED JUROR
QUALIFICATION FORM AS INSTRUCTED MAY BE SUMMONED BY
THE CLERK OR JURY COMMISSIONER TO APPEAR BEFORE THE
CLERK OR JURY COMMISSIONER TO FILL OUT A JUROR
QUALIFICATION FORM.

(C)       JUDGE MAY QUESTION JUROR ABOUT
QUALIFICATIONS.

WHEN A PERSON APPEARS FOR JURY SERVICE, OR IS
INTERVIEWED BY THE JURY JUDGE, CLERK, OR JURY
COMMISSIONER, THE PERSON MAY BE REQUIRED TO FILL OUT
ANOTHER JUROR QUALIFICATION FORM IN THE PRESENCE OF
THE JURY COMMISSIONER OR THE CLERK OF THE COURT, AND
AT THAT TIME, IF IT APPEARS WARRANTED, THE PERSON MAY
BE QUESTIONED, BUT ONLY ABOUT HIS RESPONSES TO
QUESTIONS CONTAINED ON THE FORM AND GROUNDS FOR HIS
EXCUSE OR DISQUALIFICATION. THE CLERK OR JURY
COMMISSIONER SHALL NOTE ANY ADDITIONAL INFORMATION
THUS ACQUIRED ON THE JUROR QUALIFICATION FORM AND
TRANSMIT IT TO THE JURY JUDGE.

REVISOR'S NOTE: This section presently appears as
part of Art. 51, §5(a), but was separated
from §8-205 because of subject matter
differences. This section deals exclusively
with the determination of a juror's
qualifications.
