(2) The clerk of the board shall mail all such authorizations to the
board of the proper county or Baltimore City. Upon receipt of this
authorization from the forwarding county or Baltimore City, the
clerk of the board upon a comparison of the voter's signature with
his signature as it appears on the registration records shall remove
the voter's registration from the records and retain the cancellation
authorization in the records of said board.

(c) The cancellation authorization required by subsections (a)
(2) and (b)(l) of this section shall be on a form substantially as
follows:

CANCELLATION AUTHORIZATION

I hereby apply for and authorize the cancellation of my previous

voter registration in the State of Maryland, in the county of................,

district................, precinct................, or Baltimore City, Ward............

................, precinct................. I am registered therein under the name

of........................................................(name given here must be the one

under which voter was formerly registered), my sex is................, my

date of birth is........................, my registration address therein being

............................................, and my party affiliation being.....................

My present address is.............................................................., county

or city.................................................., district or ward..........................,

precinct...............................

I intend to affiliate with the........................Party in........................

county or Baltimore City.

I do not intend to affiliate with any political party in......................

county or Baltimore City.

I am aware that a change of party affiliation within six months
of any election is prohibited by law and I declare that my decision
to cancel my registration is not related in any way to an effort on
my part to change my party affiliation.

Sec. 2. And be it further enacted, That this Act shall take effect
June 1, 1971.
