(2)    That the person is a qualified voter at the residence address

(3)    If the person voted at the preceding election, the residence
address from which he voted; and

(4)    That the voter expects in good faith to be unable to vote in
person for any of the reasons set out in Section 27-1 of this article.

27-5.

(b) (1) The application for an absentee voter who is absent from
the State of Maryland or who is unable to vote in person because of
physical disability as defined in Section 27-2 of this article, either of
whom is entitled to be registered pursuant to Section 3-7 of this
article, shall be in the following form:
