I. MILLARD TAWES, Governor                       1059

(f) The Commissioner shall require, and every agent, broker
and solicitor shall file with the Commissioner, in such form
as he may
direct, the agency or trade names to be used and the busi-
ness and residence addresses of each individual licensee, and every
agent, broker or solicitor shall promptly notify the Commissioner
of any change in the agency or trade name or in his
business or
residence address.

(E) THE COMMISSIONER SHALL REQUIRE, AND EVERY
AGENT AND BROKER SHALL FILE WITH THE COMMIS-
SIONER, IN SUCH FORM AS HE MAY DIRECT, THE AGENCY
OR TRADE NAMES TO BE USED AND THE BUSINESS AND
RESIDENCE ADDRESSES OF EACH INDIVIDUAL LICENSEE,
AND EVERY AGENT OR BROKER SHALL PROMPTLY NO-
TIFY THE COMMISSIONER OF ANY CHANGE IN THE AGEN-
CY OR TRADE NAME OR IN HIS BUSINESS OR RESIDENCE
ADDRESS.

169. Application for License.

(a) An applicant for any original license required by the provi-
sions of this subtitle shall file with the Commissioner a written appli
-
cation which shall be signed and verified by the oath of the appli-
cant. As a part of or in connection with such application, the appli
-
cant shall furnish information concerning his identity, personal his-
tory, experience, business record, purposes for which the license is
desired and any other pertinent facts which the Commissioner may
by regulation require.

169. APPLICATION FOR LICENSE.

(A)   THE COMMISSIONER SHALL PRESCRIBE AND FUR-
NISH SUCH FORMS IN CONNECTION WITH APPLICATIONS
FOR, ISSUANCE, RENEWAL OR TERMINATION OF LICENSES
AND APPOINTMENTS AS HE DEEMS APPROPRIATE.

(B)   ALL SUCH APPLICATIONS SHALL BE ACCOMPANIED
BY THE APPROPRIATE FEES IN THE RESPECTIVE
AMOUNTS STATED, WHEN REQUIRED BY THE APPLICABLE
PROVISIONS OF THIS ARTICLE.

(b) If the application is for an agent's license, such application

shall state the kinds of insurance business or subdivision thereof
proposed to be transacted, and shall be accompanied by written veri-
fication of appointment of the applicant as agent by an authorized
insurer, subject to issuance of the license.

(c) If the application is a solicitor's license, the application
shall be accompanied by a written verification of appointment of the

applicant as solicitor by his agent or broker, subject to issuance of

the license.

(d) All such applications shall be accompanied by the appropriate
fees in the respective amounts stated; when required by the appli-
cable provisions of this article.

170. Issuance and Renewal of Agents, Brokers and Solicitors AND
BROKERS Licenses.

(a) Licenses issued to agents shall expire on June 30th following
the date of issue and brokers' and solicitors' licenses shall expire
