the following entries appear with reference to him: (Here
note all entries)".

Such certificates of removal may be issued by the respective
Boards of Supervisors of the several counties and of Balti-
more City, or by a clerk designated by the said respective Boards
or by the Boards of Registry. If at the time that application
is made the name of the said voter has already been erased
from the registry, such certificate shall be issued to the voter
himself, who may apply either in person or by mail or to any
other person making application therefor. If at the time that
such application is made the name of the said voter is still upon
the registry as a qualified voter, such certificate shall be granted
and the voter's name stricken from the registry only in the
event that the said voter shall either file a written affidavit
or personally appear and take and subscribe an oath, which
affidavit or oath shall be substantially as follows:

«I ——————— do solemnly swear (or affirm) that I now
reside at ———— in the city of ————-, county of —————;
that I am the same person who is entered by that name as a
qualified voter on the registry books of the ————— precinct
of the ————— ward or district of ————— city or county;
that I have removed from the said last mentioned residence
and now reside in ————— precinct of the ——————— ward
or district of —— - — city or county; that I am entitled to
registration in the precinct to which I have removed and I
therefore request that my name be erased from the registry
of the ————— precinct of the ————— ward or district of
————— city or county, and that a certificate of removal be
mailed me at my present address as stated above (or furnished
me at this time)".

Upon receipt of an application properly executed, a certifi-
cate of removal shall be mailed to the applicant within not
less than ten days from the receipt of such application, pro-
vided that said application shall be received at the office of
the Board not later than fifty days before the next succeeding
election and provided that said application is accompanied
by a self-addressed and properly stamped return envelope.
It shall be the duty of the Boards of Supervisors of Elections
throughout the State to furnish, upon the request of any reg-
istered voter, a suitable blank form for requesting a certifi-
cate of removal as herein provided.

31. All persons who shall remove into any county of this
State or into the City of Baltimore from any other State, dis-
trict or territory of the United States shall indicate their
intent to become citizens and residents of this State by either
(a) registering their names in a suitable record book to be
procured and kept for that purpose by the clerk of the Circuit
