capable of profiting by further educational instruction as
provided in Section 33 38, and a Co-ordination Certificate
shall be issued to a minor who is participating in a school-
work Co-ordination plan as provided in Section 40 39.

24. (a) The person authorized to issue General, Vacation,
Special and Co-ordination Certificates shall not issue such
certificates until he has received, examined, approved and
made a record of the following papers, duly executed, viz:

(b) The school record of the minor, if requested "by the
Department of Labor and Industry, properly filled out and
signed, without charge to any minor applying therefor, by
the superintendent or teacher in charge of the school or
schools attended by the minor.

(c) Except as provided in sub-section (g) of this section,
a certificate signed by a physician appointed by the Commis-
sioner stating that the minor has been examined by him and
in his opinion the minor is in sufficiently sound health and
physically able to be employed in the occupation for which a
permit is applied.

(d) Evidence of age showing that the minor is of the re-
quired age for the job he intends to enter which shall consist
of one of the following proofs of age and shall be required in
the order as follows:

(1) A duly attested transcript of the birth certificate filed
according to law with a registrar of vital statistics, or other
officer charged with the duty of recording births, which
certificate shall be prima facie evidence of the age of the
minor.

(2) A passport or a duly attested transcript of a certificate
of baptism showing the date of birth and place of baptism of
the minor.

(3) A bona fide contemporary record of the date and place
of the child's birth kept in the Bible in which the records of
the births in the family of the child are preserved, a pass-
port showing the age of the child, a certificate of arrival in
the United States issued by the United States immigration
officers and showing the age of the child, or a life insurance
policy; provided, that such other satisfactory documentary
evidence has been in existence at least one year prior to the
time it is offered in evidence; and provided further that a
school record or a parent's, guardian's or custodian's affi-
davit, certificate or other written statement of age shall not
be accepted except as specified in paragraph (4).
