(b) The post office address including street number if any, at which
the business is to be conducted.
(c) The market or markets in or in connection with which the busi-
ness of the applicant is proposed to be conducted.
(d) Facts showing that the applicant has adequate personnel and
facilities to properly conduct the business for which a license is sought.
(e) A certificate from the health department of the political sub-divi-
sion in which the applicant desires to engage in business to the effect that
the applicant has complied with and is complying with all applicable sani-
tary regulations of said health department; but the Commission may
require in lieu thereof or in addition thereto the certificate to the same
effect of the State Department of Health. Where the applicant proposes
to engage in business at more than one plant, or place, separate certificates
shall be furnished and may be required for each such plant or place.
(f) A statement showing the number of delivery vehicles which the
applicant if granted a license will use.
(g) The license fee or fees required to be paid by this Article.
(h) A schedule of wholesale and retail prices which the applicant is
paying and charging for each grade, quantity and class of milk at whole-
sale and retail, respectively, and in addition thereto the applicant shall in
his application consent to the jurisdiction of the Commission in estab-
lishing wholesale and retail prices in the market or markets specified in
his application.
(C) In the case of each distributor such application shall also include
and be accompanied by the following:
(a) Either a bond or the cash equivalent in such amounts as the Com-
mission may prescribe with surety satisfactory to the Commission con-
ditioned for the payment of all obligations to producers within forty-five
days after the receipt of milk; and to secure the prompt payment of all
other sums properly required by the Commission to be paid; or
(b) A financial statement showing evidence satisfactory to the Com-
mission to the effect that the applicant is of sufficient financial responsi-
bility to insure payment for forty-five days' supply of milk, and to secure
the other payments referred to in the preceding paragraph.
(c) A schedule of the prices now paid to producers from whom milk
is to be received for sale in each market by the applicant.
(d) A statement as to the number and location of each distributing
plant which the applicant, if granted a license, will operate; and if the
applicant has been engaged in business, a statement as to the monthly
average number of pounds of milk distributed at each such plant during
the preceding year, or part thereof, during which he was so engaged; and
if the applicant has not been previously engaged in business; an estimate
as to such monthly average to be so distributed during the period of the
license applied for.
(D) In the case of each person conducting a retail store such applica-
tion shall also include or be accompanied by a statement as to the number
