[(b)] (C) Upon receiving an application (provided
under subsection (a) of this section] accompanied by a
fee in an amount established by the Board, the Board
shall issue a temporary [certificate of registration]
LICENSE which entitles the applicant to practice
audiology or speech pathology (for a period ending eight
weeks after the conclusion of the next examination given
after the date of issue] UNDER SUPERVISION WHILE
COMPLETING THE LICENSURE REQUIREMENTS.
[(c) No temporary certificate of registration shall
be issued by the Board under this section unless the
applicant shows to the satisfaction of the Board that he
is or will be supervised and trained by a person who
holds a valid certificate of registration or certificate
of endorsement issued under this subtitle.
(d) If a person who holds a temporary certificate
of registration issued under this section does not take
the next examination given after the date of issue, the
temporary certificate shall not be renewed unless there
are extenuating circumstances in the judgment of the
Board.
(e) If a person who holds a temporary certificate
of registration issued under this section takes and fails
to pass the next examination given after the date of
issue, the Board may renew the temporary certificate of
registration for a period ending eight weeks after the
conclusion of the next examination given after the date
of renewal. Three renewals may be allowed, but in any
event the time for which an individual may hold a
temporary certificate of registration shall not exceed
twenty—four (24) months. The fee for a renewal shall be
established by the Board.]
(D) THE TEMPORARY LICENSE SHALL BE EFFECTIVE FOR
ONE YEAR AND MAY BE RENEWED ONCE ON BOARD APPROVAL.
797.
(a) A person who holds a [certificate of
registration or a certificate of endorsement] LICENSE
shall notify the Board in writing of the address of the
place or places where he engages or intends to engage in
the practice of audiology or speech pathology. THE
LICENSEE SHALL NOTIFY THE BOARD IN WRITING OF ANY CHANGE
OF ADDRESS WITHIN 30 DAYS AFTER THE CHANGE.
(b) The Board shall keep a record of the places of
practice of [persons who hold certificates of
registration or certificates of endorsement] LICENSES.
(c) Any notice required to be given by the Board
to a [person who holds a certificate of registration or
certificate of endorsement] LICENSEE may be given by
mailing it to him at the address of the last place of
practice of which he has notified the Board.
