beginning with the words "A SCHEDULE" in line 16 down to and includ-
ing the word "TRANSFERRED." in line 30, and insert in lieu thereof
the following: "A SCHEDULE OF ALL INCOME, INCLUDING CAP-
ITAL GAINS, RECEIVED DURING THE YEAR FOR WHICH THE
STATEMENT IS FILED FROM ANY INDIVIDUAL PERSON OR
BUSINESS ENTITY WHICH DOES BUSINESS WITH THE STATE.
