1730                             Laws of Maryland                      [Ch. 671

seven (7) days before the election. The application shall contain an
affidavit, which need not be under oath but which shall set forth
the following" information under [upon] penalty of perjury:

(1)  The voters name and residence address, including the street
and number, if any, or rural route, if any;

(2)  That the person is a qualified voter at the residence address
given;

(3)  If the person voted at the preceding election, the residence
address from which he voted; and

(4)  That the voter expects in good faith to be unable to vote
in person for any of the reasons set out in Section 27-1 of this
article.

27-5.

(b) (1) The application for an absentee voter who is absent from
the county or Baltimore City or who is unable to vote in person
because of physical disability as defined in Section 27-2 of this
article, either of whom is entitled to be registered pursuant to
Section 3-7 of this article, shall be in the following form;

Baltimore City) Absentee Ballot for the Election to be held on
