ized agent shall mean the voter's employer, supervisor, attorney, or
member of the voter's immediate family. Any emergency absentee
ballot received by the board after the time for closing of the polls
shall not be counted.

(b) Any otherwise qualified voter who is blind, physically dis-
abled or who has impaired vision and is for one of these reasons un-
able to mark his absentee ballot and sign the required oath, may be
assisted in voting by any person selected by the voter. Any person
rendering assistance pursuant to this subsection shall execute the
certification set forth in Section 27-8 (h) of this article.

27-4.

(a) A qualified voter desiring to vote at any election as an absen-
tee voter shall make application in writing to the board for an absen-
tee ballot, which application must be received not later than [ten
(10)] seven (7) days before the election. The application shall con-
tain an affidavit, which shall set forth the following information
upon penalty of perjury:

(1)  The voter's name and residence address, including the street
and number, if any, or rural route, if any;

(2)   That the person is a qualified voter at the residence address
given;

(3)   If the person voted at the preceding election, the residence
address from which he voted; and

(4)  That the voter expects in good faith to be unable to vote in
person for any of the reasons set out in Section 27-1 of this article.

27-5.

(a)  Printed forms of applications for absentee ballots in accord-
ance with the requirements of this subtitle shall be provided by the
boards and shall be available to [anyone] any qualified voter upon
request.

(b)   (1) The application for an absentee voter who is absent from
the [State of Maryland] county or Baltimore City or who is unable
to vote in person because of physical disability as defined in Section
27-2 of this article, either of whom is entitled to be registered pur-
suant to Section 3-7 of this article, shall be in the following form:
