(ii) Three representatives of insurance camera that offer health
care coverage in the commercial market; and

(iii) Three consumer members who receive, or have a family
member who receives, mental health care services.

(5)     The following members appointed by the Maryland Insurance
Commissioner:

(i) One representative of the commercial health insurance industry;

(ii) One representative of a commercial health maintenance
organization; and

(iii) One representative of the managed behavioral health care
industry;

(6)     The following members appointed by the Secretary of Health and
Mental Hygiene:

(i) One representative of the Maryland Hospital Association;

(ii) One representative of the Mental Health Association of
Maryland;

(iii) One representative of the Community Behavioral Health
Association of Maryland;

(iv) One representative of Maryland's pediatric mental health
community;

(v) One consumer member who is the family member of an adult
who receives mental health care services;

(vi) One consumer member who is the family member of a child who
receives mental health care services; and

(vii) One consumer member who is a direct consumer of mental
health services; and

(7)     One member representing the rural mental health community
appointed by the FORVM for Rural Maryland.

(5) The following members appointed by the Maryland Insurance
Commissioner:

(i) One representative of the commercial health insurance
industry;

(ii) One representative of a commercial health maintenance
organization; and

(iii) One representative of the managed behavioral health care
industry; and
