coordination of benefits form adopted by the Insurance Commissioner as the sole
instrument for determining nonduplication or coordination coverage with other
health insurance policies or contracts under which the insured, member, or
subscriber may make a claim; prohibiting an insurer, nonprofit health service plan,
or health maintenance organization from imposing certain additional requirements
as a condition of reimbursement; requiring every nonprofit health service plan and
insurer to use the uniform summary explanation of benefits form adopted by the
Insurance Commissioner as the sole instrument for providing the summary
explanation of benefits to an insured requiring laboratories to accept a certain
uniform laboratory referral form as the sole instrument for referrals for laboratory
services and prohibiting laboratories from imposing certain requirements; requiring
every health maintenance organization, nonprofit health service plan, and insurer to
use the uniform provider voucher form adopted by the Commission as the sole
instrument for providing the explanation of payment for medical benefits to a
provider; requiring the Insurance Commissioner to adopt by regulation a uniform
coordination of benefits form, a uniform summary explanation of benefits provider
voucher form, a uniform laboratory referral form, and a uniform consultation
referral form; requiring insurers, nonprofit health service plans, and health
maintenance organizations to use certain forms under certain circumstances;
prohibiting insurers, nonprofit health service plans, and health maintenance
organizations from imposing certain requirements as a condition of coverage;
requiring certain forms to be properly completed; requiring the Commissioner to
consult with certain persons when developing the certain forms; requiring certain
forms to be capable of electronic transfer; requiring the Insurance Commissioner to
assess the use of certain forms in other states and throughout certain industries in
developing certain forms; requiring certain provisions of this Act to be implemented
by a certain date; and generally relating to health care cost containment and certain
uniform forms.
