(1)  The identity of the person to whom the
[liability] DEBT was owed and the date it was incurred;

(2)  The amount of the [liability] DEBT owed as
at the end of the year for which the statement is filed;

(3)  The terms of payment of the [liability] DEBT
and the extent to which the principal amount of the
[liability] DEBT was increased or reduced during the year;
and

(4)  The security given, if any, for the
[liability] DEBT.

(g) List of family members employed by State. —A list
of all members of the immediate family of the person making
the statement employed by the State in any capacity at any
time during the year for which the statement is filed.

(h) Source of earned income. — The name and address
of each place of salaried employment and of each business
entity of which the person or a member of his immediate
family was a sole or partial owner and from which the person
or a member of his immediate family received earned income,
at any time during the year for which the statement is
filed.

(i) Additional information. — Such additional
interests or information as the person making the statement
might desire.

4-104. Interests attributable to person making statement.

For the purposes of Section 4-103(a), (b), and (c) and
the disclosures therein required, the following shall be
considered to be the interests of the person making the
statement:

(a)  Any interest held by the spouse or a child of the
person making the statement, if such interest was at any
time during the year for which the statement is filed
directly or indirectly controlled by the person making the
statement.

(b)  Any interest held by a business entity[,] in which
[business entity] a 30 percent or greater [equity] interest
was at any time during the year for which the statement is
filed [an interest of] HELD BY the person making the
statement.

(c)  Any interest held by a trust[,] or an estate in
which, at any time during the year for which the statement
is filed, [(1)] the person making the statement. (1) held a
reversionary interest or was a beneficiary[,] or (2) [if a
revocable trust, with respect to which the person making the
statement] was a settler IF THE TRUST WAS REVOCABLE. A
